HTN Monitoring

• Main point of contact for patients that are a part of the RPM Program
• Help develop Hypertension Care Plan with patient and provider
• Establishes realistic and achievable goals for blood pressure control with the patient and Provider.
• Assist patients in signing the RPM Consent form and ensure that it is attached to their chart
• Provides education and demonstration of BP devices in clinic
• Ensure that patients are meeting the eligibility requirements for RPM enrollment
• Work across multiple platforms to ensure documentation control and device inventory
• Review readings 2 to 3 times per day
• Maintain narratives for each review or patient contact in comments section of Unity
• Communicate with patients regarding readings that are outside normal parameters
• Readings within 140/90 – 180/120 to be followed up with phone call from RPM Case ICD code I10
• Managers
  • Has patient missed a dose of medication
  • Any changes in patient status
• Schedule follow up visits for RPM patients with uncontrolled HTN
• Provides detailed documentation of all patient encounters and all communication with patient
• Send encounters to higher providers regarding patient communication or medication issues
• Attach PDF RPM files to eCW for follow up visits
• Follow up with patients who have not had a reading in several days
• Assist in identifying eligible patients for Community Paramedicine

Educational/Experience Requirements:

• NC EMT–Paramedic or National Registry Paramedic
• Registered Nurse
• 3+ years experience in a clinical setting

• $25/hour
